Brezis’s review points to multiple studies conducted between 2002 and 2023 that indicate a “significant increase” in depressive symptoms and suicidal behavior among finasteride users. “Over 20 years worldwide, hundreds of thousands may have endured depression, and hundreds may have died by suicide,” Brezis stated.

Users of finasteride have reported persistent symptoms even after discontinuing the medication, including insomnia, panic attacks, cognitive dysfunction, and suicidal thoughts. Brezis emphasizes that the evidence is no longer anecdotal, stating, “We now see consistent patterns across diverse populations. And the consequences may have been tragic.”

The FDA has acknowledged the potential for depression as a side effect of finasteride since 2011, adding suicidality to the list in 2022. However, Brezis argues that the number of suicides linked to the drug could be in the thousands, far exceeding the 18 reported cases in 2011.

A Call for Regulatory Action

Brezis urges regulators to enforce stricter requirements for ongoing post-approval studies to ensure the safety of medications like finasteride. He warns that many physicians prescribe the drug without fully understanding its risks, often leading to dire consequences for patients.

“My recommendation to patients and physicians: Stay away from this medication,” Brezis cautioned. “My recommendation to the FDA: Take it off the market.”

Manufacturer’s Response

Finasteride, originally developed by Merck and marketed under the brand names Proscar and Propecia, is now produced by Organon, which stands by the drug’s safety and efficacy. In a statement, the company emphasized that regulatory agencies have thoroughly reviewed the medication’s safety data and continue to monitor it as part of routine post-marketing surveillance.

“Organon encourages patients to speak with their doctors if they have any questions or concerns about their health or medication,” the statement read.
